Annual Report 2025-2026
Annual Report Submission
The deadline to submit the Annual Report is June 12th, 2026 5PM EST.
This Annual Report can serve as a request for renewal, there will be a section for you to indicate such in this report. This report can take an average of 1-1.5 hours to complete. Based on convener feedback, since Qualtrics does not allow you to save your progress, we have provided questions below so you can prepare your responses and copy them into the survey form. The Annual Report must be submitted through Qualtrics.
The report contains the following questions:
- Indicate your seminar title and convener names/affiliations.
- What challenges/barriers did you face throughout AY 25-26?
- Please provide an overall assessment of your seminar during AY 25-26, including the extent to which your seminar accomplished its intended outcomes and met or did not meet CS requirements.
- How did your seminar contribute to or support student success at UNC during AY 25-26?
- How many attendees did your seminar have during AY 25-26?
- Using this template, provide an overview of meetings/events during AY 25-26. Once completed, prepare the event template report as a PDF with the naming convention: MeetingEventOverview25-26_[Seminar Name], to be uploaded to the Qualtrics survey.
- Upload a single PDF with marketing materials used this year, if any.
For traditional seminars seeking renewal only:
- Any changes to seminar titles or conveners?
- How will your seminar continue to prioritize student success?
- How will your seminar continue to improve or change?
- What are your seminar’s objectives for AY 25-26?
- What new products or outcomes will you hope to achieve by the end of AY 25-26?
For FieldNotes working groups only:
- How did your FieldNotes group work to build (intellectual and/or social) community? If so, what strategies were helpful for building community? If not, what were some impediments or challenges you faced?
- How might you recommend future FieldNotes groups might build community in light of these successes or challenges?
- How did your group decide to use its funds over the course of AY 25-26? You do not need to share exact dollar amounts, but rough estimates are helpful.
Traditional Seminars
Traditional seminars seeking renewal for the following academic year will not be considered or receive funding until an Annual Report is received. Annual Reports must be received before the deadline.
Problem-based Seminars
Failure to submit an Annual Report for problem-based seminars will result in a pause in funding.
Notifications
We will notify conveners regarding their renewal/continuation status in early August. Please do not contact Carolina Seminars regarding your seminar status before then. We encourage conveners to please submit their reports in a timely manner to help quicken the review process.
