We have provided the following document as a template for the Annual Report. This document provides the following points that we would like seminars to address as you reflect on the past year. Please ensure you are addressing these key points in your assessment however, you may expand and provide more insight beyond the points given.
In this document, we also request that seminars provide a brief overview of each event/meeting held throughout the year. Please follow the format provided.
Once completed, please fill out the form below and upload the Annual Report as a PDF.
We encourage seminars to also submit any promotional materials used for events.
The deadline to submit an Annual Report is June 4th, 2021 5PM EST.
